A woman has died after a vehicle being followed by police was involved in a crash early this morning (November 25) on the A13 near Rainham.

Essex Police said the vehicle failed to stop when requested by officers just after 2.30am.

The vehicle being pursued was the involved in a serious collision with another vehicle.

A female passenger in the second vehicle died, and a London Ambulance spokesperson said two other people involved were taken to hospital.

The A13 westbound, from the M25 to the junction with the A1153 in Dagenham, remains closed due to the crash.

Following the incident, the force referred itself to the Independent Office for Police Conduct (IOPC).

Chief Supt Stuart Hooper said: "We are currently working with our colleagues in the Metropolitan Police Service who, at this stage, are leading the response to the incident.

"As is appropriate, we have made a mandatory referral to the Independent Office for Police Conduct which has declared an independent investigation."

A spokesperson for the Met said it continues to manage the scene and liaise with Essex Police and the IOPC.
