A confidential death document, which details the last moments of a young man’s life, has turned up on a grass verge in Romford.
Now an investigation has been launched by the London Ambulance Service (LAS), who say they are taking the loss “very seriously”.
A member of the public found the two-page document on the grass outside Romford Ice Rink, in Rom Valley Way, Romford. The witness statement – a legal manuscript – may have been included in evidence during the young man’s inquest. In it, an Ilford emergency medical technician gives a highly-detailed account of the moment he and a female colleague are called to the man found hanged.
The passerby who found the document said: “Due to the sensitive nature of the report I do not think it should be for public reading and the person who was responsible for its safe-keeping should have ensured this was the case.”
An LAS spokesman: “We take any loss of confidential information very seriously. We have started an investigation into how it occurred and we will take any action necessary to prevent it happening again.”
